1. The application forms of the Residency and Fellowship Training may be obtained from the Medical Services Department. Completed forms along with required documents will be forwarded to the chairman of the department applied for.
2. All applications for residency training shall be reviewed by the residency committee of each department where the applicant wishes to train. The composition of such committee shall be made at the discretion of the department chairman. The committee shall be empowered to do the following:
- administer written examinations,
- conduct interviews and
- two (2) weeks pre- residency training/observational (if applied for residency program) as methods of screening the applicants and choosing the successful and qualified candidates
3. Each residency committee shall forward its recommendations to the respective department chairman.
4. The department chairman shall endorse to the Deputy Executive Director for Medical Services (who is also the Chairman, Medical Services Credentials Committee [MSCC] ) the eligible / qualified applicants. Accomplished evaluation of the qualified applicants shall be attached. The Deputy Executive Director for Medical Services / Chairman, MSCC shall recommend for approval the recommendees of the department chairmen, (subject to the availability of residency plantilla item – if for plantilla position).
5. The Office of the Executive Director shall approve the appointments.
6. The Department Chairmen shall notify the successful applicants of their appointment.
7. The appointee shall report to the Human Resource Management Division (HRMD) for submission of additional requirements (#12). All legal requirements including PE and Psychological Test result must be complied with prior to the start of training.
8. The appointee shall report to the HRMD (3 rd Floor) for psychological evaluation / test and Industrial / Employees Clinic for his pre-employment Physical Examination which shall be scheduled by the HRMD.
9. All successful applicants shall be briefed on hospital policies by the Medical Services Department, and specific departmental policies by the department chairman or the chief resident prior to the start of the training. They shall also be required to attend the orientation program for incoming residents / fellows.
10. When all the requirements have been completed and reviewed by the Human Resource and Management Division and/or the Office of the Deputy Executive Director for Medical Services, the appointee shall sign the temporary appointment for residency / fellowship (if with plantilla item) contract for residency / fellowship (if without plantilla item) at the HRMD. This shall be recorded as the official commencement of residency training. All records will be held on file by the HRMD.

- Certificate of Internship (Resident applicant) / Certificate of Residency (Fellowship applicant)
- Recommendation letter regarding internship performance (Resident applicant) / Recommendation letter regarding residency performance (Fellow applicant)
- Passport-size picture (1 copy)
12. Requirements for successful candidates (appointee)
To be submitted in original or authenticated xerox copies before signing the temporary appointment.

- ID picture 1 pc. (2x2) & 2 pcs. (1 ½ x 1 ½) colored with blue background
- Certificate of Live Birth or Notarized affidavit of two disinterested person if birth certificate is not available (original copy)
- Marriage Contract (if married)
- Children’s Birth Certificates
- Professional Tax Receipt (PTR) – current year
- PRC card
- NBI Clearance (original copy)
- Certificate of good moral character from two (2) disinterested persons – references preferred (original copy)
- Narcotic License Number (S2)
13. No appointee may go on duty in the hospital until all requirements are completed and the contract is signed.
