Requirements for New patients to be admitted in the Regular Dialysis Program |
| |
- official copy of the HbSAg and Anti HCV test
- dialysis prescription from the attending Nephrologist
- Secure a permanent hospital number at admitting section and present to reception staff for charging purposes.
- must have an attending Nephrologist from NKTI for private patients.

For traveling patients: (additional requirement)

- at least one week notice for the schedule and confirm a day before arrival to the Center.
- Present a medical abstract or a copy of 1 treatment sheet from previous dialysis center.
